If you are using Google Chrome the fields will be visible but you will not be able to fill it in and save it so you

Once you have filled in the form either email to Registration Secretary and make a Direct Deposit or post to Registration Secretary with cheque or money order.

INFORMATION FOR OVERSEAS REGISTRANTS
Overseas Registrants purchasing orchids at the conference may take their
plants home to their countries.

The Conference Committee will establish an office to assist registrants
in exporting their plants.

1. Registrants will need to provide receipts covering their purchases to
the office with a list of the plants.
2. The CITIES permit and the Application for Inspection for Phyto will
be issued at the show for a fee of $120. (payable in cash). An Approved
Inspection site  and time of inspection will need to be nominated by the
registrant.  (we will have a list of Approved Inspection sites at the show)
3. The data entry will be carried out off site and a confirmation
certificate will be issued at the show.
4. The plants will need to be taken to an approved inspection site
during the following working week.
